I keep your personal data purely to assist in providing hypnotherapy and other related services to you.
At Clifton Village Hypnotherapy (Sophie West) I am committed to maintaining the trust and confidence of anyone that contacts me via email/text/website/phone call or social media.
Anything discussed by email, by phone or in a session is confidential. There are two instances where I would share your information: With emergency services or you GP if I genuinely thought that you were likely to harm yourself or someone else. Storage of data:
I will have your email address and/or phone number. I store emails appointment times and notes I have made during your appointment(s) and keep them for my records. This information is stored on password protected computers.
Any data recorded on paper is kept in accordance with GDPR regulations.
If you phone or text me then I will have your phone number and also the information in the text(s) stored on my phone. This information will be included in your record stored on computer. I also have access to some emails and my calendar on my phone. The phone is password and fingerprint protected.
The information is kept so that I know what we have discussed, have records of financial transactions and have information to use if you do business with me again.
Your rights:
You have the right to request a copy of the information that is held about you. If you would like a copy of some or all of your personal information, please contact me. Please be aware that my session notes are usually handwritten.
You also have the right to request that I delete any or all of the date I have related to you.
However, I may not be able to immediately comply with this request as my insurance company’s advice is to retain records for a minimum of 6 years after the last treatment (or 6 years after reaching the age of 18 for a minor) and some insurance companies require data to be kept for a specified minimum number of years. I may also need to keep some data for tax purposes.
IMPORTANT: Please note that the effects of therapy may differ for different clients, full
client commitment is important. No therapy can give you a 100% guarantee of a cure. All
clients are required to complete a questionnaire form before undertaking hypnotherapy, in
which you will be asked to disclose certain medical information. Whilst clinical
hypnotherapy is very safe if you are undergoing psychiatric treatment, then you should
obtain a letter from your medical practitioner to confirm that you may have hypnotherapy.
Payment is due at the end of each session either by cash/card /cheque.
Cancellations require 48 hours notice or a 50% fee will be incurred. However, we recognise that challenging situations do arise, and you will always be treated fairly.
